NEW YORK, NY, January 21, 2012 /24-7PressRelease/ -- A newly released Buildings Department report shows there were numerous elevator violations at the office building where an advertising executive was crushed in a Manhattan, New York elevator accident on December 14.
According to the New York Post (1/12/2012), city inspectors cited 11 violations against 13 of the building's elevators immediately after the fatal New York elevator accident.

The elevator involved in the fatal elevator accident in New York had been cited for lacking a "certificate of compliance," reported the NYP. However, other elevators had more serious problems.
The New York Post said, one elevator's cable was not lined up with a groove on a pulley that moves the cable. Other violations included, the use of nails instead of cotter pins "on shackles meant to hold heavy elevator car counterweights in place," and improperly attached cable clamps.
